Bicep tenodesis/transposition in pets - Indications, surgery & recovery

Biceps tenodesis is a surgical technique used in pets with severe shoulder instability. The procedure is performed primarily in dogs, and in exceptional cases in cats. The goal of the surgery is to relocate the biceps tendon, giving it a new stabilizing role in the shoulder joint.

In this article, we discuss the indications, the surgical technique, recovery, and prognosis after a biceps tenodesis.

What is a biceps tenodesis?

Normally, the biceps tendon runs through a groove at the front of the upper arm (the intertubercular groove, or sulcus intertubercularis). In a bicep transposition, the tendon is surgically relocated to a new position, usually on the medial side of the joint. This allows the tendon to contribute to stability and relieve the load on damaged structures.

When is a biceps tenodesis necessary?

This surgery is considered for pets with severe medial shoulder instability.

Which animals are at risk?

biceps tenodesis is mainly performed in dogs. In cats, the procedure is extremely rare and is only carried out in exceptional cases, usually following severe trauma.

The surgical technique

During the surgery, the biceps tendon is released from its normal groove and reattached in a new location using bone anchors or tunnels. This requires great precision to position the tendon at the correct tension and location. The procedure is performed under general anesthesia and is often combined with arthroscopic examination of the joint to identify any additional injuries.

Recovery and aftercare

After a biceps tenodesis, a strict rehabilitation protocol is crucial. The first few weeks consist of crate rest and controlled short walks. After 4 to 6 weeks, physiotherapy and light muscle-strengthening exercises can begin. Hydrotherapy is a valuable addition for building muscle strength without overloading the joint.

Full recovery can certainly take up to 6 months. The prognosis is generally good, especially with carefully performed surgery and consistent aftercare.
